Am I alone in thinking that it would be really useful to have a basic guide
to using Windows 10 with a screen reader?

Many things are the same, but I feel that it would be helpful to have a
brief setup guide, together with a piece  detailing some of the differences
encountered when upgrading to 10.

No, I don't think those options are in Windows 10.
When you're in a list of files, press TAB. You should find yourself on the
column headings - your screen reader should report these as split buttons.
>From here, left or right arrow a few times until you land on "Name"
(assuming you're not already there).
Press SPACE or ENTER on "Name" and you should be taken back to the list of
files, sorted alphabetically.
If it's sorted the wrong way round, repeat the steps again.
I'm not sure whether this applies to all folders or just the active one.

I don't seem to have those options in Win 10 - although I did in win 7.  

Are you using Win 10 as you have described?

Open the folder you want. Alt-v for view then downArrow. Make sure details
is checked. Continue down the view options. You'll get arrange by and in
that submenu, you'll see name. If you continue down, you'll get ascending or
descending.
